Alcohol withdrawal timeline: hours, days, and when to go to an ER

This is not a home-detox plan

Alcohol withdrawal can cause seizures and delirium tremens. Those are emergencies. If someone has a history of withdrawal seizures, is medically fragile, or is already confused or hallucinating, call 911 or go to the nearest emergency department. The timeline below is how clinicians teach risk windows. It is not a promise of what your body will do.

Rough windows clinicians still teach

6–12 hours after the last drink: anxiety, tremor, nausea, insomnia, and a rising heart rate can start. Some people already feel too sick to travel without help.

12–24 hours: risk of withdrawal seizures begins to matter, especially with prior seizures or very heavy daily drinking. This is a common window where “waiting until morning” goes wrong.

24–72 hours: hallucinations and, in a smaller group, delirium tremens — severe confusion, autonomic instability — can appear. DTs are a hospital problem.

After 72 hours: many people are through the most dangerous autonomic phase, but sleep, mood, and blood pressure can stay wrecked. That is often when a residential setting is about food, monitoring, and the next level of care — not about “being done.”

What CIWA-Ar is (and what we are not claiming)

Hospitals and some medical units score alcohol withdrawal with tools such as the CIWA-Ar (Clinical Institute Withdrawal Assessment for Alcohol, revised). That score guides medication in settings that are licensed and staffed to give it.
